The specialist who ordered a test three years ago is not the one reading the result today, and the only person holding the whole picture is usually you, working from a folder and from memory, inside a fifteen-minute appointment.
Strata Integration exists to fix that transfer. Your complete history is assembled and reconciled into one document, so that the next decision about your health can be made against the entirety of your data rather than the part of it that happened to be in the room.
This phase requires your active participation. We review your available medical records, laboratory reports, and medication lists, and conduct a structured interview to map your health journey across time.
Structure
A four-week engagement including up to 8 analysis hours, of which up to two are live virtual sessions.
Session 1: Health history mapping. We clarify your timeline, review available laboratory results, and identify patterns worth discussing with your physician.
Session 2: Synthesis and forward organisation. We bring your history and laboratory trends into one structured document, identify the Health Flags that warrant physician follow-up, and build your 90-Day Framework.
Between sessions, records are reviewed and reconciled, and your questions are answered in writing.
What You Receive
Your Clinical Health Biography, a permanent and editable document in three parts.
Health Narrative: your history mapped into a coherent timeline across every provider involved, with laboratory results from different systems and countries reconciled into one comparable set of values, and cross-system patterns identified.
Clinician's Summary: the part that does the transferring. A concise section written to be read by a doctor who has never met you, including up to five Health Flags with suggested questions for follow-up.
90-Day Framework: a structured plan you can follow between appointments, built from your own data and adjusted to your circumstances.
The document is yours to keep. Bring it to every appointment, send it ahead of a new referral, and carry it with you when you change providers, insurers, or countries.
What Happens Next
Your Framework runs for ninety days. The Integration engagement itself is four weeks, which means you carry the remaining two months on your own. Many clients prefer to, and the document is written so that you can.
